Frequency of anal cytology abnormalities in women with high grade intraepithelial squamous lesion in genital tract

The natural history of human papillomavirus (HPV) infection appears to be similar in the epithelium of the female genital tract and the anal epithelium. Certain groups have a higher susceptibility to the development of pre-neoplastic lesions, such as patients living with HIV and those with high-grade intraepithelial lesions in the genital tract. Objective: To evaluate the frequency of abnormalities in anal cytology among women with high-grade squamous intraepithelial lesions of the cervix and patients living with HIV receiving follow-up at the Clinics Hospital of the Faculty of Medicine of Ribeirão Preto, University of São Paulo (HCFMRP-USP). Methods: A retrospective observational study included women followed at HCFMRP-USP from January 2016 to December 2021. A total of 300 medical records were selected, of which 57.66% underwent anal lesion screening due to abnormalities in pap smear or cervical biopsy indicating high-grade lesions (CIN II or CIN III), and 42.33% were women living with HIV. Anal cytological abnormalities were observed in 4.66% of the participants. Results: Anal cytological changes were found in 3.3% of women with high-grade cervical intraepithelial neoplasia and in 7.97% of the women who underwent anal cancer screening due to living with HIV. Conclusions: This study observed a high prevalence of HPV infection and HPV-induced lesions in the anal canal of women with high-grade cervical neoplasia and HIV-positive patients, confirming a positive association only between anal cytological changes and HIV seropositivity.
