Current management of vulvar intraepithelial lesions associated or not with human papillomavirus (HPV)

When evaluating vulvar intraepithelial lesions, whether or not associated with human papillomavirus (HPV), it is necessary to assess the significance of these alterations, such as the risk of progression to vulvar cancer, symptoms, and impact on sexuality. Definitive diagnosis is made by biopsy. High-grade squamous intraepithelial lesions occur in younger patients with risk factors similar to those associated with cervical intraepithelial neoplasia (high-risk HPV, smoking, and multiple sexual partners). Treatment should be individualized and depends on the patient's age and the clinical characteristics of the lesions. Vulvar intraepithelial lesions not associated with HPV are differentiated intraepithelial neoplasia, associated with vulvar dermatoses and occur in older patients. Treatment is excisional.
